Renal risk and response in amyloidosis.

چکیده
In this issue of Blood, Palladini and colleagues address incidence of and risk factors for end-stage renal disease (ESRD) and proffer a new renal response system for patients with amyloid light-chain (AL) amyloidosis using a test set of 461 patients from Pavia, Italy, and a validation set of 271 patients from Heidelberg, Germany. Validation of organ response criteria has trailed behind the progress made in the field of AL amyloidosis in diagnosis, prognosis, and hematologic response, but Palladini and colleagues' study helps breach this gap.

A staging system for renal outcome and early markers of renal response to chemotherapy in AL amyloidosis.
The kidney is involved in 70% of patients with immunoglobulin light-chain (AL) amyloidosis, but little is known on progression or reversibility of renal involvement, and criteria for renal response have never been validated. Newly diagnosed patients from the Pavia (n = 461, testing cohort) and Heidelberg (n = 271, validation cohort) centers were included. Proteinuria >5 g/24 h and estimated glo...

Immunoglobulin light-chain (AL) amyloidosis is caused by a usually small plasma cell clone synthesizing light chains undergoing conformational changes that lead to their aggregationanddeposition in tissues. This process leads toprogressivemultiple organdysfunction anddeath if left untreated.Advanced, irreversibleorgandamageatpresentation is the main limitation to improve theoutcomeofpatientswit...

متن کاملA Rare Presentation of Multiple Myeloma: A Case Report of Hepatic Amyloidosis
The clinically reported case of liver involvement with multiple myeloma (MM) is rare. Amyloidosis, defined as a tissue deposition of clonal light-chain fibrils, has been reported in 10-15% of the MM patients. We described a rare MM patient with the primal presentation of fulminant hepatic failure and biliary system involvement due to amyloidosis. Our patient had the primal symptoms of hyperbili...
متن کاملAbattoir study of the prevalence of renal lesions in slaughtered cattle
This study was carried out to determine the prevalence and type of renal lesions in cattle that were slaughtered at the Ghaem (Shahriar) and Ziaran (Ghazvin) abattoirs. A total number of 405 clinically normal cattle were randomly selected and renal samples were taken for histopathological study. The cattle were divided into two groups according to sex, and female groups were divided into two ag...
